CS responsibilities include:
    - Case management: Link by integrating, connecting and monitoring consumers for mental health, medical and dental service; document all functions on CS service notes;
    - Supportive counseling: Encourage self-management of symptoms and behavior, promote integration into community, monitor and document progress
    - Skill building in social and job situations to increase independence and self management.

John Santucci is Club Nova's Coordinator of Community Support Services.  John is in charge of developing Person-Centered Plans (PCPs).  PCPs are required for a person to receive community support and are used by all different types of community service agencies, not just Club Nova.  Please see "becoming a member" to learn more about PCPs in relation to Club Nova.
